Senior Living Help After a Hospital Stay in Cape Coral & Fort Myers
When the discharge planner says “they shouldn’t go home alone,” call us early. We help families move quickly without settling for the wrong place.
What to do first
- Ask the discharge planner or case manager for the expected discharge date and the care level they recommend.
- Ask whether rehab in a skilled nursing facility is recommended, or whether assisted living is appropriate.
- Request a copy of recent physician orders and a medication list.
- Call us. Same-day calls matter when openings are limited.
Rehab first, or straight to assisted living?
Medicare generally covers a limited stay in a skilled nursing facility for rehab after a qualifying inpatient hospital stay (generally three days as an admitted inpatient, not “observation”). Medicare does not pay for assisted living rent or personal care. Ask the hospital whether your loved one was admitted or under observation, because it affects coverage. Details: Medicare.gov SNF coverage.
How we help when time is short
- We focus only on communities likely to have an opening and able to meet the care needs
- We help coordinate the health assessment communities require before move-in
- We work alongside your loved one’s hospital or rehab case manager
- We explain costs up front so there are no surprises
